STUDENT INFORMATION CENTER

 

Students are responsible for printing OCLC class handouts and bringing to classes taught at OCLC sites.

OCLC Evaluation Forms --

Please print these forms and bring to class.

 

ISBE forms for CPDU credit and ISBE evaluation are available for printing at:

Evaluation for Workshops, Conference, Seminar, etc. (77-21A)
Evidence of Completion for Workshops, Conference, Seminar, etc. (77-21B)

These students will only need F77-04 (claim for credit) and F77-21(the eval form).
